[C.CC USERS] Book updated, mostly

Hi all,

The book has been updated a bit. A new section on "Getting Started"
addresses some of the issues that a few of you have experienced.

My next project is to update screenshots and process networks later in
the text to reflect some of the corrections. Specifically, making it
so diagrams are, wherever possible, generated as part of the build of
the book. This will also allow me to generate both a print and
electronic copy, which will help in terms of book size for non-print
reading.
